You can use any office. You do not have to go to the one in your own borough.

Do you even need to go in person?

Often, no. HRA's brochure on the Medicare Savings Program (MSP) says you can apply in person or by mail, and that you can apply for Medicaid and/or the MSP on ACCESS HRA. HRA's Health Assistance page says the same for Medicaid.

  • Go in person if you want someone to look at your papers, you need a paper application today, or you want a date-stamped copy right away.
  • Mail it if you already have the form and copies of your proofs. For MSP-only, HRA's brochure lists: HRA/Medical Assistance Program, Initial Eligibility Unit, 5th Fl., PO Box 24390, Brooklyn, NY 11202-9814. See our MSP mailing guide for the other address and for counties outside NYC.
  • Use ACCESS HRA (nyc.gov/accesshra) if you are comfortable online and can upload photos of your documents.
  • Call first if you are not sure. The HRA Medicaid Helpline is 1-888-692-6116.

Step by step: a visit that works the first time

  1. Pick the office and call it. Use the phone number in the table. Ask if it is open that day and if you need anything extra.
  2. Decide what you are applying for. MSP-only uses the short state form DOH-4328. Medicaid (with or without MSP) uses the longer application. If you are not sure, say so at the window and ask them to check you for both.
  3. Bring copies, not originals. HRA says to send copies, and that you need proof of residency, identity, income, and immigration status. HRA now also requires proof of Medicare.
  4. Arrive early in the day. Most offices open at 8:30 or 9:00 a.m. and close at 5:00 p.m.
  5. Get proof you applied. Ask for a receipt or a date-stamped copy of the first page. Write down the name of the worker and the date.
  6. Watch your mail. For the MSP, the state instruction sheet says you get a Notice of Action within 45 days of filing.

What to bring if you have Medicare

  • Your red, white, and blue Medicare card (copy of front and back).
  • Photo ID or proof of birth date, such as a New York driver's license, U.S. passport, birth certificate, or green card.
  • Proof of address dated in the last six months, such as a lease, rent receipt, letter from your landlord, or a utility bill.
  • Proof of all income: your Social Security award letter for this year, pension statements, and pay stubs if you work.
  • Proof of any health premiums you pay, like a Medigap or retiree plan bill. These can lower your countable income.
  • Immigration papers if you are not a U.S. citizen, such as an I-551 green card.
  • Bank statements only if you apply for full Medicaid. The MSP in New York has no resource test, but full Medicaid does.

Common mistakes

  • Going to the Kings County Hospital site. HRA still lists it as closed until further notice (a phone appears on the same page—use another Brooklyn office or ACCESS HRA).
  • Bringing originals and leaving them. Bring copies.
  • Forgetting proof of Medicare. HRA says it is now required.
  • Applying only for Medicaid when you also want help with your Part B premium. Ask them to check you for the MSP too.
  • Leaving without proof of the date you applied.

Want a rough sense of which cost program to ask about?

The MSP screener takes a few questions (county, household size, approximate monthly income) and shows an educational estimate. It does not decide eligibility and does not enroll you in anything.
