In short: yes. New York's own MSP form instructions say: "You will need to renew (recertify) your MSP every year." You do not file a brand-new application. You answer a renewal notice. Some cases renew automatically when the state can check your information electronically, but you should still watch your mail. If no notice comes, call your local Medicaid office (HRA in New York City) and ask.
Last checked September 28, 2026, against the DOH-4328 instructions, the state's MSP 2026 page, NYC's renewal FAQ, and HRA's March 2026 Medicaid Alert on renewals. Immigration-status section added October 5, 2026, from the state notice GIS 26 MA/14.
Who sends your renewal, and when
Who sends the renewal depends on where your case lives. Since late 2025, the state has been moving people with Medicare, people over 65 who are not seeking long-term care, and MSP applicants to NY State of Health (the state's online enrollment system). Others stay with HRA or their county Department of Social Services (DSS). Your last approval notice names the office handling your case.
| Your case is with | When the notice comes | How you can renew |
|---|---|---|
| NYC HRA | HRA mails renewal notices 3 months before the renewal date (NYC renewal FAQ). | Mail back the packet in the envelope, call HRA's FE-ABD Program at 347-396-4705, renew on ACCESS HRA, or bring the packet to an HRA office. |
| NY State of Health | Notices go out 30 days or more before the renewal date, plus emails or texts about 45 days ahead. | In your NY State of Health online account, or with the Customer Service Center at 1-855-355-5777. |
| A county DSS outside NYC | Your county mails the notice. | Follow the notice. Call the county DSS if anything is unclear. |
HRA's renewal form for an MSP-only case is the MAP-909F ("Medicare Savings Program Renewal Notification"). If you also have full Medicaid, you will usually get a different renewal form, such as the MAP-909E, instead.
Step by step: renewing your MSP
- Open every letter from HRA, your county, or NY State of Health. Look for words like "renewal," "recertification," or "respond by." Write the respond-by date on your calendar.
- Read what it asks for. Sometimes you only need to confirm your information. Other times it asks for proof of income, such as this year's Social Security letter.
- Gather copies of what it asks for. Never send originals.
- Send it back one way, and keep proof. Use the return envelope, the phone, ACCESS HRA, or an office. Keep a copy of every page you send and write down the date.
- Check that it went through. In NYC, call the HRA Medicaid Helpline at 1-888-692-6116. HRA's renewal card says to choose your language, press 2 for the client Medicaid menu, press 1 for case information, then press 1 for renewal status. Have your date of birth, case number, and/or Social Security number ready.
- Watch your Social Security payment. If your Part B premium starts coming out of your check again, your MSP may have stopped. Call right away.
What if no renewal notice ever came?
The DOH-4328 instructions say that if you do not get a renewal notice, contact your local Medicaid office. In NYC, that is HRA.
- Check ACCESS HRA first (NYC). HRA's March 2026 alert says that if the respond-by date has passed, or it is within 90 days after your case closed, you can print your renewal form from the E-Notices tab. Click "E-Notices," then "View notices from the last year," then open the PDF. The MAP-909F MSP renewal is one of the forms you can print there.
- Don't count on "Print Medicaid Renewal Form" for MSP-only. HRA says that button only makes the MAP-909E and MAP-2096F. It cannot reprint the MAP-909F.
- Send the printout by fax, in person, or by mail. HRA says all three work.
- No ACCESS HRA account? Call 1-888-692-6116 and ask the agent to have the renewal form mailed to you.
- Outside NYC: call your county DSS and ask them to mail a new renewal form.
Deadlines, and how to avoid a gap
- HRA cases: the NYC renewal FAQ says that if you renew within 30 days after the renewal date, there is no gap in coverage.
- NY State of Health cases: the same FAQ says that if you renew by the end of the month after the renewal due date, there is no gap.
- If your MSP closed anyway, send the renewal (or a new application) right away and ask the office how to restore it. Our renewal letters guide and our denial and fair hearing guide explain the next steps.
QI is a little different
The state's MSP 2026 page says the QI program is paid for by a yearly federal allotment, while QMB is not. In practice, this is one more reason to renew on time and not let a QI case lapse. Our QMB vs QI guide explains the two programs.
Immigration status at renewal (from October 2026)
Since October 1, 2026, HRA and county offices must check immigration status when they decide MSP, under the state notice GIS 26 MA/14. MSP is now limited to U.S. citizens and nationals, green-card holders who have finished or are exempt from the 5-year wait, Cuban-Haitian entrants, and COFA migrants. Income limits did not change.
If your renewal asks for immigration papers, send copies by the deadline. The state notice says that when a requested document never comes back, Medicaid must be denied or stopped. If you lost MSP because of your status, see what happens next.
What to have ready
- Your renewal notice and your case number.
- Your Social Security award letter for this year, and pension statements if you have them.
- Pay stubs if you still work.
- Proof of any health premiums you pay, like a Medigap or retiree plan bill.
- Your Medicare card.
- Any letter about a change: a new address, a death in the household, or a marriage.
Common mistakes
- Thinking MSP lasts forever once you are approved. It has to be renewed every year.
- Waiting for a letter that went to an old address. Tell HRA, your county, or NY State of Health when you move.
- Using the ACCESS HRA "Print Medicaid Renewal Form" button for an MSP-only case. Check E-Notices instead.
- Not keeping proof of what you sent and when.
- Ignoring a Part B premium that shows up on your Social Security payment again.

The MSP screener takes a few questions (county, household size, approximate monthly income) and shows an educational estimate. It does not decide eligibility and does not enroll you in anything.
