- NY Connects: 1-800-342-9871 (Relay 7-1-1). Useful in this neighborhood because so many households are also managing long-term services.
- Nearest in-person site: India Home, 178-36 Wexford Terrace, Jamaica, NY 11432, 917-288-7600, weekdays 9 to 5, by appointment. The Q17 runs down 188th Street to Hillside Avenue near the site. Book through NYC Aging Connect at 212-244-6469 (ask for HIICAP) and ask which site is easiest by bus from your block.

Both spouses' cases are HRA's. The HRA Medicaid Helpline is 1-888-692-6116. If one spouse already has a Medicaid case, mention that case when calling about the other; it helps HRA find the household.
What usually happens next
- Understand the household rule. For a Medicare Savings Program, a married couple living together is a household of two and both incomes count, even if only one spouse is applying. The state's standards chart says an MSP household is always one or two.
- Compare the couple's combined monthly income to the couple limits. For 2026, health.ny.gov lists $2,489 a month for QMB and $3,355 for QI. New York applies no resource test to either program, so the house and savings do not enter into it.
- Notice that the spouse with home care may already have QMB through the Medicaid case. That does not automatically extend to the other spouse; the second spouse usually needs an application of their own.
- Call HIICAP with both Social Security letters and any pension statements and ask them to walk through the couple math.
- File DOH-4328 with HRA for the spouse without coverage, with copies of the Medicare card, ID, income proof for both spouses, and an address document.
Common mistakes
- Applying as a single person because "only I am applying." For an MSP, a married couple living together is assessed on combined income. The couple limits are higher, but the math must include both.
- Assuming the spouse with Medicaid and home care covers the other spouse's Medicare premium. Each spouse has their own Medicare, and each needs an MSP on their own record.
- Skipping the counselor because there is no train. The Q17 reaches Hillside Avenue near India Home, and the phone line reaches the same counselors from your kitchen table.
